A case of Graves' disease accompanied by pheochromocytoma presenting with normal levels of catecholamines

Introduction: Susceptibility to Graves' disease is determined by a mixture of genetic, environmental and endogenous factors. The mechanisms involved have not yet been elucidated. Although Graves' disease is often accompanied by other autoimmune diseases, only a few cases of Graves' disease accompanied by pheochromocytoma have been described. Six cases of pheochromocytoma and concurrent Graves' disease have been reported so far. The association between pheochromocytoma and Graves' disease suggests that Graves' disease may be induced by excessive catecholamine secreted by a pheochromocytoma. We report herein a case of a patient with Graves’ disease who presented with a right adrenal tumor incidentally detected on chest computed tomography (CT) scans and was diagnosed with a pheochromocytoma, which showed normal urine and serum levels of catecholamines and their metabolites. Case Presentation: A 45-year-old woman was referred to our hospital for the evaluation of a right adrenal incidentaloma detected by chest computed tomography (CT). She was diagnosed with Graves' disease 1 month ago. She had no symptoms of pheochromocytoma such as hypertension or a history of hypertension attack. A regular shaped mass (dimension 48 X 36mm) with a heterogenous and solid structure was diagnosed in Abdomen computed tomography (CT). Two consecutive 24-hour urine samples were sent for measurement of catecholamines, both of which showed normal levels of free cortisol, metanephrine, vanillylmandelic acid (VMA), epinephrine and norepinephrine. The patient was given a diagnosis of a non-functional adrenal incidental lesion and underwent right adrenalectomy. Histopathological data correlated with pheochromocytoma as well. This case suggests that the relationship of Graves' disease may be associated with excess catecholamine secreted by pheochromocytoma. In addition, although the conventional method for detecting pheochromocytoma is to identify an increase in urine catecholamines, physicians should be aware of the possibility of false negativity.

부신전이암으로 오인된 부신우연종과 이에 병발된 뇌하수체 우연종 1예

우연종이란 임상적으로 관련이 없는 진단적 검사나 치료도중 의도하지 않게 우연히 발견되는 종양으로 최근 컴퓨터단층촬영과 자기공명영 상이 발달하면서 발견 빈도가 증가하고 있다. 우연종이 발견되었을 때는 반드시 기능성 병변 여부를 판별하여야 하고 종양 크기 및 증상 유무를 확인하여 양성 및 악성인지를 감별해야 한다. 본 저자들은 악성을 시사하는 임상경과를 취하였으나 양성선종으로 판명된 부신우연종 과 이에 병발된 뇌하수체 우연종을 경험하였기에 이에 보고하는 바이다. 증례: 75세 남자환자가 간세포암으로 진단 받고 2차례의 경동맥화 학색전술 치료 후 외래 추적 관찰 중이었다. 진단 당시 복부컴퓨터단층촬영 검사에서 우연히 직경 1.3cm 가량의 왼쪽 부신종양이 발견되었 으나 1년 이상 크기의 변화가 없어서 경과 관찰하던 상태로 최근 6개월 사이에 부신종양의 크기가 1cm 이상 급격하게 증가하고 종양주변부 로 조영 증강이 뚜렷하게 관찰되어 전이성 병변의 가능성을 강력히 시사하였다. 저자들은 간세포암의 부신전이와 기능성 부신선암종의 감별 을 위해 기능성 부신종양에 대한 선별검사들을 시행하였고 이 가운데 24시간 요중 유리 코티솔치가 260.6 mcg/day 로 높게 측정되어 저용량 과 고용량 덱사메타손 억제검사및 뇌하수체호르몬 검사와 뇌 자기공명영상촬영을 시행하였다. 저용량과 고용량 덱사메타손 억제검사결과와 뇌하수체호르몬들의 기저치는 정상으로 나왔고 뇌하수체에 직경 5mm의 우연종이 발견되었다. 결국 부신종양 및 뇌하수체종양은 비기능성 종양인 것으로 밝혀졌지만 간세포암의 부신전이 가능성 때문에 확진 및 치료를 위해 개복하여 왼쪽 부신절제술을 시행하였다. 조직검사상 양성 부신선종으로 밝혀졌으며 현재 환자는 회복되어 외래 추적 관찰중이다.
