A case of hepatocellular carcinoma that caused extrahepatic lung metastasis without intrahepatic recurrence after hepatic resection

Hepatocellular carcinoma (HCC) is one of the most common cancers worldwide. Early diagnosis and curative hepatic resection is important to improve the survival rate. However, longterm survival after curative resection remains low because of high recurrence rate after resection.
A 59-years-old man was received the S7,S8 hepatic segmentectomy due to hepatocellular carcinoma without evidence of metastasis. 50 month later, there were two pulmonary nodules on chest CT and we found left endobronchial mass by bronchoscopy. Transthoracic needle biposy confirmed lung metastasis of primary hepatocellular carcinoma. We report a case with extra-hepatic recurrent of hepatocellular carcinoma to the lung without primary hepatic recurrence after hepatic resection.

Paclitaxel loaded polymeric micelle(Genexol-PM®) 투여 후 발생한 허혈성 대장염 1예

Paclitaxel have been widely used for treatment of many solid tumors as a combination chemotherapeutic regimen. Although colonic toxicity is an unusual complication of paclitaxel-based chemotherapy, the reported toxicities include pseudomembranous colitis, neutropenic enterocolitis, and rarely ischemic colitis. Recently developed Genexol-PM® - a cremophor-free, polymeric micelle-formulated paclitaxel – shows more potent antitumor effect because it can increase the usual dose of paclitaxel due to not including toxic cremophor compound. We report a case of a 57-year-old man with advanced non-small cell lung cancer who developed ischemic colitis 5 days after second cycles of chemotherapy with polymeric micelle-formulated paclitaxel and cisplatin. The patient complained of bloody diarrhea with abdominal pain on left lower quadrant. His white blood cell count was within normal range. Colonoscopy revealed continuous circular diffuse mucosal hemorrhage and edema from sigmoid colon to splenic flexure(Figure 1B). After the bowel rest by maintenance of fasting state with total parenteral nutrition for 12 days, his symptoms were recovered and follow-up colonoscopic finding showed healing state of mucosa. Since then, he was treated by pemetrexed monotherapy instead of paclitaxel compound and platinum.
