2 Department of Dentistry, Ajou University School of Medicine, Suwon, Korea Introduction: This study examined the effect of autogenous tooth bone used as a graft material for bone regeneration in an artificial bony defect of minipigs. Materials and Methods: Four healthy minipigs, weighing approximately 35-40 kg, were used. Four standardized artificial two-walled bony defects, 5 mm in length and depth, were made on the bilateral partial edentulous alveolar ridge on the mandible of minipigs, and autogenous tooth bone was augmented in the right side as the experimental group. On the other hand, only alloplastic bone graft material HA was grafted with the same size and manner in the left side as the control group. All minipigs were sacrificed at 4 weeks after a bone graft and evaluated histologically by Haematoxylin-eosin staining. The specimens were also evaluated semi-quantitatively via a histomorphometric study. The percentage of new bone over the total area was evaluated using digital software for an area calculation.

Comparative evaluation of three calcium phosphate synthetic block bone graft materials for bone regeneration in rabbit calvaria

In the present study we assessed the effectiveness of three types of calcium phosphate synthetic block bone graft material in bone regeneration using defects in the rabbit calvarium. Rabbit cranial defect models have been used in numerous studies for evaluating newly developed biomaterials due to the adequate amount of bone marrow. An 8-mm defect is known to be smaller than the critical defect size in rabbits for evaluating reossification, but has been suggested as a useful defect model for determining the effects of osteoinduction. The use of four 8-mm defects allowed comparison of an early-phase healing response for several materials, while avoiding individual variations. In the present study, the late and intermediate healing responses were analyzed at 8 and 4 weeks postsurgery, respectively.

A slowly resorbable biomaterial might be suitable in sub-sinusal bone augmentation for preventing the re-expan- sion process and for augmenting the density of the regen- erated tissues. Bovine HA seems to be the most efficient fillers for 3 dimensional stability of sub-sinusal bone aug- mentation. 48 In this study, AutoBT and Bio-Oss showed similar healing pattern, excellent graft volume maintenance and active new bone formation, in microCT and histomor- phometric evaluation of biopsy specimen 4 months after graft procedure. So, if it was applied for sinus bone graft, it would be useful material struggling repneumatization against intra-sinus pressure. Bone formation was found sur- rounding both enamel and dentin portion of AutoBT material. New bone bridges between graft materials were also detected in AutoBT group. Also newly-formed and matured lamellar bone was detected surrounding AutoBT material, osteoblasts covering newly-formed bone accumu- lated osteoid.

Usually, an autogenous bone graft shows high bone resorption requiring more harvest volume at the donor site and a second op- eration [21]. Although AutoBT possesses osteoconductive and os- teoinductive potential in that the healing process is very similar to that of free autogenous bone grafts, it showed successful results in maintaining the graft volume until a year after surgery in the pres- ent study. The highly crystalline structure of the enamel portion in the AutoBT powder probably caused a slow resorption of the graft- ed material with a relatively slow remodeling process.

IV. Discussion Since the lack of alveolar bone is one of the most prevalent and difficult problems to address in implant surgery, many methods have been developed and used. In the past, autogenous onlay bone graft was mainly used for the impro- vement of horizontally and vertically shrunken alveolar ridge. Note, however, that it needed additional surgery on the donor site, and predicting the result was difficult since bone resorption varied according to the collection area 5 . Recently, the trend of research reports on the selection of standard treatment for knife-edge ridge has been changing slightly 7,8 . Simion et al. 3 performed vertical bone augmentation with demineralized freeze-dried bone or autogenous bone chip and non-resorbable membrane and reported a good result after 1-5 years’ follow-up. Another paper reported that horizontal bone augmentation with the mixture of autoge nous bone and xenobone covered with resorbable membrane resulted in 4.6 mm bone gain. Another paper reported 3.6 mm bone gain when only xenobone was used in horizontal bone augmentation 7 . AutoBT was developed and clinically applied from 2008 in Korea. Effect on bone formation of the autogenous tooth graft in the treatment of peri-implant vertical bone defects in the minipigs

Bone grafts are being made using materials extracted from autogenous teeth. These materials have excellent biocompatibility without any immune reactions or foreign body reactions because they are autogenous tissues devel- oped by taking note of the fact that the constituents of teeth are similar to those of bones, involve no risk of dis- ease infection, and are not physiologically rejected by pa- tients. When extracted teeth are demineralized, organic components are removed and the inorganic component hydroxyapatite remains as a main component. The com- ponents of tooth bone, which is a graft material made by using teeth, have already been experimentally verified in many studies and papers [4-9]. However, those studies are different from the present study, as most of them are in regard to tooth bone made from other persons’ teeth already extracted after removing all organic components for immunological safety and safety against infection trans- mission. The present study approached the effectiveness of autogenous tooth bone grafts that have the same benefits as autogenous bone grafts in that rejection of transplant can be fundamentally blocked because autogenous tooth bone, immunologically speaking, has the same origin as au- togenous bone, so that the possibility of the transmission of a source of infection between different persons or spe- cies can be fundamentally blocked.

Abstract (J Korean Assoc Oral Maxillofac Surg 2013;39:120-126) Objectives: This study sought to elucidate the effect of autogenous tooth bone material by experimenting on minipig's maxillary sinus and performing histological and histomorphometric analyses. Materials and Methods: Five 18-24 month-old male minipigs were selected, and right maxillary sinuses were grafted with bone graft material made of their respective autogenous teeth extracted eight weeks earlier. The left sides were grafted with synthetic hydroxyapatite as control groups. All minipigs were sacrificed at 12 weeks after bone graft, which was known to be 1 sigma (σ) period for pigs. Specimens were evaluated histologically under a light microscope after haematoxylin-eosin staining followed by semi-quantitative study via histomorphometric analysis. The ratio of new bone to total area was evaluated using digital software for calculation of area.

Comparison of immunohistochemical analysis on sinus augmentation using demineralized tooth graft and bovine bone

In this study, the expression levels of type I collagen, os- teopontin, and osteocalcin were significantly increased at eight weeks relative to at two weeks in both groups. These results suggest that both demineralized human tooth bone and Bio-Oss material exhibit similar osteogenesis and osteo- conduction capabilities in the rabbit maxillary sinus. To our knowledge, this is the first comparative immunohistochemi- cal analysis to show that demineralized particulate tooth bone exhibits more active bone regeneration in comparison with anorganic bovine bone. Therefore, demineralized particulate tooth bone, as an autologous biomaterial, might be used as a bone graft material for reconstruction of bony defects in im- plant dentistry and oral and maxillofacial surgery.

Jensen et al. 12 compared the bone healing achieved using autografting, anorganic bovine bone-derived mineral (ABBM), and synthetic b-tricalcium phosphate (b-TCP) in intraosseous defects in the mandible of minipigs. Autografting resulted in faster bone regeneration than grafting with either ABBM or synthetic b- TCP in the early healing phase. Although newly formed bone ultimately regenerated in all defects after 8 weeks regardless of the graft material used, autografting resulted in increased osseous maturity during all healing periods. Other studies have also shown that autogenous bone can induce faster and more mature bone regeneration in the initial healing period of lateral ridge augmentation 13,14 . In spite of these advantages, the unpredictable resorption rate, limited availability, and patient morbidity limit its common use. Bone substitutes may overcome these potential shortcomings, and a good solution when autogenous bone is harvested may be combining that harvested bone with an osteoconductive bone substitute 15 .

Introduction The autogenous bone graft had been used for the reconstruction of maxillofacial hard tissue defect, but the disadvantage such as morbidity of donor site 1) . The development of xenogeneic bone and synthetic bone have been made in order to overcome this limitation, but the problem such as the delay of the bone remodeling, immune rejection was raised1. Thus, recently, the research about the autogenous teeth graft material using extracted tooth was actively performed and the clinically good result was reported in many researches 1, 2) . Also, the concern with the autogenous teeth graft material has been growing, patients try to utilize extracted tooth as the autogenous teeth graft material. The autogenous teeth graft material is made by mainly using the 3rd molar, sound tooth which is extracted with the periodontal problem, extracted premolar for the orthodontic treatment. Kim 3) reported the good bone revision by using the autogenous teeth graft material using the premolar in the bone graft for implant surgery. Thus, the author paid attention to the possibility as the autogenous teeth graft material of the supernumerary tooth, not the 3rd molar or other teeth.

2 Four types of graft materials for replacing hard tissue are used in GBR procedures; autogenous or autografts, allografts, xenografts and alloplasts. Among these, autograft is considered a gold standard being a source of the most osteogenic organic material. It is however not without limits, such as donor site morbidity and limited dimensions of graft obtainable 3 . To overcome these advantages, other types of natural transplants and synthetic materials have been developed for mass-production. Xenografts are non-human source materials obtained from another species and are widely used in clinical applications. Next to the bovine-derived bone mineral, the use of porcine bone graft tissue has been introduced by several researchers based on its structural and physiological compatibility between human and swine bone tissue 4,5 . Deproteinized porcine bone mineral (DPBM) has shown comparable bone regeneration and volume stability when used in maxillary sinus graft and ridge preservation after tooth extraction 6,7 .

Comparative study of new bone formation capability of zirconia bone graft material in rabbit calvarial

Histomorphometric analysis of the total bone formation area showed that significant differences among all groups existed for every period (P < .05). This difference was due to new bone formation as well as the different porosity of the bone graft materials. The Zi bone graft images showed an insufficient distance for new bone ingrowth between the granules compare to those of other bone graft materials, and this seems to be associated with the shape of the Zi granules. Unlike other granules that have a spherical shape, the Zi graft material was atypical, with an irregular form and had sharp edges that could densely fill the spaces between granules. If zirconia bone graft material could be produced as spherical granules with similar shapes, better comparisons could be made with other experimental groups. Recent stud- ies have documented the formation of new bones through zirconia surface treatment, and bone grafting with zirconia could be developed more effectively. 48-50

Familial tooth bone graft for ridge and sinus augmentation: a report of two cases

Through a treatment process similar to that used for autog- enous tooth bone-graft materials, the teeth extracted from im- mediate family members were preserved, treated, and stored. Tooth material was prepared as bone graft material; this could be donated, distributed, and grafted to the family for therapeu- tic purposes. Here, the term “family” refers to brothers, sisters, parents or children. The extraction and the tooth to be donated were teeth that could not be saved by dental treatments, the deciduous teeth, and the wisdom teeth. When a tooth is ex- tracted from a family member and donated or received, the dental clinic obtains the individual’s consent for donation as well as consent for transplantation. The consent is signed by the patients themselves or guardians, and a written agree- ment for treatment of the extracted tooth is signed. Donors are submitted to basic tests for venereal diseases and acquired immunodeficient virus if requested by the donor or the recipi- ent. The recipient should prepare a document that proves the family relationship and sign agreement to transplantation 6 .
