1. GENERAL ITEMS

1.1. Introduction

These Special Provisions shall be read and construed in conjunction with, and shall take precedence over the listed and otherwise applicable "Standard Specifications for Highway Construction (most recent edition)”, referred to in this document as 'Standard Specifications', and the “Forest Service Bridge Design and Construction Manual (most recent edition)”, referred to in this document as ‘Bridge Manual’.

Payment for work will be made at the stipulated prices bid for the items appearing in the Major Works Contract Schedule A. Any work called for which is not listed as an item in Schedule 'A' will not be paid for separately.

1.2. Prime Contractor Identification

In addition to Article 12.02 of the Major Works Contract, Safety Conditions and Prime Contractor Agreement, the contractor must supply and erect a sign at either end of the construction site, identifying the prime contractor information. This information must be consistent with the criteria as stated in the BC Catalogue of Standard Traffic Signs. This document can be found at:

1.3. Subcontractor and Supplier Identification

If the Contractor requires Subcontractors and/or Suppliers, The Contractor shall submit a list of Subcontractors and/or Suppliers to the Ministry for approval prior to any works being performed by the subcontractor. After award, the successful Contractor shall contract out the specified works to the identified subcontractors and suppliers. The Contractor shall notify the Ministry if there are any changes to the list of subcontractors and suppliers.

It is recommended that the list of Subcontractor and/or Suppliers be supplied to the ministry with the appropriate Schedule as listed in the Invitation to Tender Table of Contents.

The Ministry MUST approve all subcontractors/consultants. Any Works conducted by non-approved subcontractors/consultants could result in the Works being REJECTED.

1.4. Access to Site(s)

The Contractor shall be fully responsible for access to the works. The Contractor shall satisfy that access to the works is adequate to get equipment and materials to the work site(s). The Contractor shall be responsible for the cost of any temporary access works and road

maintenance that may be required.

The Contractor shall provide 72 hours of notice to the persons who are the primary maintainers of the roads before using the applicable roads that access the work site(s).

The Contractor must ensure that the equipment working on or hauling material to and from the site(s) does not damage or deposit material onto any existing roadway. Any material spilled on roadways open to public traffic or on driveways must be cleaned up immediately. It will be the Contractor’s responsibility to repair any damage to roads and driveways cause by construction equipment and/or operations.

Upon completion of the hauling portion of the contract, the Contractor must grade and repair the haul roads to the satisfaction the Ministry Representative.

Accesses to the sites are gained over the highway and Forest Service Roads. Transport vehicle(s) shall take into consideration travel on gravel forestry roads.

No separate payment will be made for the above items.

1.5. Adherence to Drawings

The various parts of the work shall be built in strict accordance with the drawings and with any supplementary drawings that may be furnished during the progress of the work as occasion requires

1.6. Layout of Work

Any Reference Points and clearing width boundaries that have been marked in the field must be carefully preserved. The Contractor must be satisfied before commencing any work, as to the meaning and correctness of all marks and stakes, as no claim will be entertained for any alleged inaccuracies. Should the Contractor discover or suspect any errors in the marks, the Contractor shall at once discontinue the affected work until such errors are investigated and, if necessary, rectified.

The Contractor shall at the their own expense provide all other survey control and layout, and shall provide assistance to the Ministry Representative in establishment of lines, grades or controls required for checking or measuring up the work as required.

In addition to the requirements of Article 22 of the Construction Agreement, the work shall be so laid out unless indicated differently on the detailed design drawings and constructed that the location of the following elements shall not differ from the true positions by more than the following amounts:

 Horizontal location of Substructure elements including:

› Piles*, Posts, Footings, Retaining Walls ± 25 mm

› *at ground or waterline ± 75 mm

 Maximum deviation from plumb or specified batter for:

› Piles 20 mm per meter

 Horizontal location of Anchor bolt group ± 15 mm

 Vertical location of bridge seats ± 10 mm

1.7. Construction Schedule

In accordance with Article 7 of the Agreement, the Contractor must submit to the Ministry Representative the Construction Schedule and a list of equipment prior to the pre-construction meeting or within seven days of acceptance of the Tender, whichever is sooner.

This Construction Schedule must meet all the contract milestone dates and also address all items of the Work that require approval or certification by the Ministry. The Ministry will require at least 72 hours’ notice for any changes to the Construction Schedule.

The costs for completing the Construction Schedule are considered incidental.

1.8. Time Schedule

The Contractor shall complete and submit a Time Schedule showing dates related to the works and milestones.

The Contractor will schedule and carry out the construction work so as to comply with work site conditions specified by the Province to be in place on the following dates in chronological order:

The Contractor at all times shall notify the Ministry of any delays or gains on the schedule. The costs for completing the Work Time Schedule are considered incidental.

1.9. Industrial Health and Safety Regulations

The Contractor must observe and enforce the construction safety measures required by the Canadian Safety Code (NRCC15562), Provincial Government, and Work Safe BC.

In the event of conflict between any provisions of the above authorities, the Contractor must apply the most stringent.

Prior to start of construction, the Contractor must give notice in writing to Work Safe BC as required in Section 20.2 of the “Occupational Health and Safety Regulations”

2. SITE SPECIFIC PROVISIONS

2.1. Location of Work

The work is located at km 85.5km on the Finlay Forest Service Road.

 Site #: G6-070

› Latitude: 55°34’18”

› Longitude: -123°44’43”

2.2. Scope of Work

The work to be carried out under this contract consists of the following:

 Prior to construction prepare:

› All Necessary environmental permits

› Environmental Management Plan

› Traffic Management Plan

› Quality Control Plan

› Proof of Division 3 or better CWB welding firm.

› ALL OF THE ABOVE ITEMS TO BE SUBMITTED TO THE MINISTY FOR ACCEPTANCE

 Construction

› Mobilization/Demobilization

› Excavation

› Removal and Disposal of existing culvert

› Stream Diversion

› Supply and Place Granular Material

› Place Rip Rap c/w required fabric;

› Stream Channel Reconstruction

› Unload and Install of one concrete slab bridge (c/w columns, spread footings, concrete ballast walls, Concrete Barriers and any other applicable

hardware/material);

2.3. Availability of Site(s)

The site(s) are available for immediate construction.

2.4. Camp Facilities

The Contractor may establish a temporary construction camp and shall obtain all necessary building and operating permits for the camp facility.

Ministry Representative shall approve any camp location prior to any preparatory work being carried out.

2.5. Drawings, Appendices and Technical Reports

The following tables contain a list of Drawings, Appendices and Technical reports that form part of this contract. The contractor shall ensure that the correct versions of all items listed are included in the contract packages.

2.6. Supply of Certain Materials by the Ministry

The Ministry will supply at no cost to the Contractor the following other materials:

 Concrete Slab Bridge c/w:

› HSS Steel Rails,

› post and Pad Abutments

› Concrete Ballast Walls

› Concrete No-Post Guardrails

› Signs and Posts including narrow structure signs (WA-24) and delineators

› Grout

› All materials will be delivered to site but the contractor is responsible for unloading the bridge materials.

 Supply of Temporary Access Bridge (optional: See Section 2.2)

2.6.1. The Ministry Structure Fabricator Contact Information

The Contractor is responsible to give 14 days’ notice in writing to the Fabricator and the Ministry Representative on when they want the bridge components delivered. This time frame is needed to help ensure the proper scheduling and transport of all the bridge components.

2.7. Supply of Certain Materials by the Contractor

The Contractor shall supply the following other materials:

 Granular Road/Bridge Approach Material

 Rip Rap

 Grass Seed

 Installation, Maintenance and Decommission of Temporary Access

 Supply of Temporary Access Bridge. (Optional: See Section 3.3.)

The Contractor shall give the Ministry 5 days’ notice to allow for inspection of materials and components. No materials or components shall be incorporated into the work until approved by the Ministry.

The Contractor shall be responsible for and bear all travel, board and lodging costs incurred by the Ministry or Consultant personnel required to inspect materials fabricated outside the Province of British Columbia and the costs shall be offset from progress, or any, payments.

2.8. Potential Gravel and Rock Sources

The following is a list identifies known possible Gravel sources complete with Contact information that may be usable by the contractor.

The contractor is responsible for obtaining approvals to use and/or any costs that may be required for the material selected for use.

2.8.1. Gravel Source

Potential Gravel Sources are from the road excavation or within the road right-of-way (if found usable)

2.8.2. Rip Rap Source

A potential rock source is located around 8km Bug Lake Road (turn off at 74km Finlay FSR).

2.9. Traffic Management Plan

A Traffic Management Plan (TMP) is to be submitted to the Ministry for review prior to any construction activities. Below are some key elements that must be addressed /considered within the TMP:

 ACCESS THROUGH THE SITE MUST BE AVAILABLE AT ALL TIMES

 Details on any Detour Roads or Temporary Crossings

› DETOUR ROAD AND CROSSING HAVE TO BE ACCEPTABLE TO:

▪ HEAVY INDUSTRIAL TRAFFIC

▪ RECREATIONAL TRAFFIC; AND

▪ RESIDENTIAL TRAFFIC

 Plan dealing with any disruptions

 District Manager notified a minimum of 72 hours before commencing construction

 Primary Road Users notified 72 hours prior to any disruption

 The Contractor shall minimize inconvenience to the road users during construction,

 The Contractor shall erect and maintain proper and adequate barricades and signs

 Any other traffic control devices are required by WorkSafeBC and the Ministry.

2.9.1. Payment Item 1.01

Section 2.9 will be paid for under Payment Item 1.01 in the Schedule A of the Major Works Contract.

Any amendments and/or requirements needed to meet the TMP shall be considered incidental to the stipulated price bid, and no other compensation shall be made to the contractor, unless otherwise approved by the Ministry Representative.

2.10. Protection of Utilities

The Contractor shall preserve any utility properties that are located within their work site(s). Any relocation of the utilities to facilitate the construction work will be the responsibility of the

Contractor. The Contractor shall also be responsible for any damage caused by construction operations to any utility properties.

The Ministry is not aware of any known utilities at the work site(s) but the Contractor should verify the presence of any utilities prior to commencing the Works.

2.11. Protection of the Environment 2.11.1. General

The stream has a Riparian Classification of S3. The Contractor shall comply with the Rules, Legislation and Regulations pursuant to the work, and with the following

requirements:

 Operations shall be altered or suspended immediately upon the request of an Enforcement Officer from the Ministry of Forests Lands and Natural Resource Operations (MFLNRO) or Ministry of Environment (MOE), or a Department of Fisheries and Oceans (DFO) Officer.

 Disturbed areas shall be revegetated, by seeding with an approved grass/clover mix, mulching with hay/straw bales and the planting of willow shoots, as directed by the Ministry Representative.

 The Contractors operations shall be in compliance with requirements of the Contractor’s Sediment and Drainage Management Plan.

 Any other conditions required by MFLNRO, DFO or MOE.

2.11.2. Maintaining Water Quality

Erosion and sedimentation are major concerns with respect to their potential impact on water quality and fish habitat. The Contractor is responsible for:

 Planning, scheduling, and performing this Work in such a manner that the quality of water flowing from the Site is at all times acceptable to officials of the DFO, MFLNRO and MOE, and will take immediate action to correct any deficiency in water quality.

 Maintaining conditions, which protect the environment not only during active construction on the Site, but also during periods when the Contractor has suspended its construction activity for any reason.

2.11.3. Environmental Management Plan

The contractor shall complete an Environmental Management Plan (EMP) prior to construction. The EMP shall cover all issues associated with in-stream and include all locations in which the work will be conducted, including but not limited to haul roads, temporary structures, borrow and granular material excavations, and disposal sites. The Contractor will not commence work in any area so identified until an EMP has been submitted and accepted by the Ministry Representative.

The EMP shall at a minimum include the following items:

 Stream Diversion Procedures

 Stream Channel Restoration

› Sediment and Drainage Management Plan:

› Schedule for the Work, as it pertains to the Plan;

› Construction procedures to limit the potential for erosion and sediment production;

› Site specific measures for runoff and drainage management;

› Site specific measures for erosion prevention and control;

› Measures for ensuring acceptable water quality at points of discharge to streams and other water bodies;

› Drawings, as required by the Ministry Representative, of mitigate measures; and

› Commitment of onsite equipment, such as water pumps, and materials for erosion, sediment and drainage control, to deal with emergency situations.

 The Contractor is responsible for any environmental permits required for the works (fish salvage, fish window variance, etc.)

Following an initial inspection with the Contractor, and thereafter from time to time as the Ministry Representative deems necessary, the Ministry Representative may identify areas for which the Contractor will be required to amend their EMP.

The Ministry Representative’s acceptance of the Contractor’s EMP will be valid only so long as the anticipated conditions upon which the Plan was based on continue to be applicable. The Contractor will at all times be responsible for ensuring that the Work is in accordance with the current Plan.

The Ministry Representative may order the Contractor to update and resubmit its Plan at any time. The Ministry Representative may order the Contractor to stop work, or to take other precautionary or remedial measures whenever the Contractor is carrying out any Work that is not in accordance with the Plan. All costs of any resulting delay will be borne entirely by the Contractor.

2.11.4. Environmental Monitor 2.11.4.1. General

The Contractor may require the services of an Environmental Monitor.

This section covers the general and specific provisions regarding the authority and responsibility of Environmental Monitors.

2.11.4.2. Definitions

For the purpose of this specification, the 'Environmental Monitor' shall mean a qualified agent with credentials and expertise acceptable to the Federal and Provincial Environmental officers. The Environmental Monitor will provide expert advice to the contractor to facilitate compliance with the environmental protection aspects of the Forest Practices Code Act, the federal Fisheries Act, permits, approvals and the Environmental Management Plan, provided for this project. Under the authority of the Ministry Representative, the Environmental Monitor will monitor the Contractor’s compliance with Department of Fisheries and Oceans and Ministry of Water Land and Air Protection requirements, including the Environmental Management Plan and approved Sediment and Drainage Management Plans.

2.11.4.3. Intent of this Specification

These specifications give the Ministry the right to exercise control over environmental aspects of the work, when the Contractor’s operations or proposed operations have a negative effect on the water quality and/or aquatic resources, as deemed, by the Ministry Representative. On advice from the Environmental Monitor, the Ministry Representative may require the Contractor to cease operations and/or propose less intrusive alternatives, as well as mitigate measures. On the approval of the Ministry Representative the Contractor will proceed with the approved alternative and/or mitigate measures.

These specifications are for the protection of the environment, and shall be given such interpretation as will secure this intent.

2.11.5. Payment Item 1.02

Section 2.11 will be paid for under Payment Item 1.02 in the Schedule A of the Major Works Contract.

Any amendments and/or requirements needed to meet the EMP shall be considered incidental to the stipulated price bid, and no other compensation shall be made to the contractor, unless otherwise approved by the Ministry Representative.

2.12. Disposal Site

Waste or other materials specified or designated by the Ministry Representative are to be disposed of at designated sites. All designated disposal site(s) are to be identified by the contractor and accepted by a Ministry Representative.

The Contractor must ensure that the disposal site complies with all Federal, Provincial, Municipal and Regional by-laws and regulations prior to disposing of the material. The Contractor will pay any fees incurred.

2.13. Quality Control 2.13.1. General

Quality Control (QC) is the process of checking specific product or service results to determine if they comply with relevant quality standards and identifying ways to eliminate causes of unsatisfactory product or service performance.

The Contractor shall carry out quality control and performance procedures to ensure that all requirements of the conceptual and detailed drawings including material/product testing, and documentation are met.

The Contractor shall provide unrestricted access during working hours to all Quality Control operations and documentation produced by or on behalf of the Contractor. The Ministry Representative will review the Contractor’s performance of the Work and

determine the acceptability of the Work based on the Ministry’s Quality Assurance results

and, where deemed appropriate by the Ministry Representative, by the Contractor’s Quality Control results

2.13.2. Quality Control Manager

The Contractor shall designate one person as the Quality Control Manager (QC Manager) who shall be responsible for the implementation of the QC Plan. The QC Manager shall be a person with knowledge, skills and abilities and be approved by the Ministry Representative. The QC Manager, unless approved by the Ministry, shall be at arm’s length from the productivity part of the Contractor’s organization and specifically shall not be the Project Manager or the Project Superintendent.

The QC Manager must be able to perform all of the QC Manager’s relevant duties and shall remain on site at all times the Contractor is performing Work which must be tested or inspected in-process, and must be readily accessible and able to return when off-Site.

2.13.3. Quality Control Plan

The Contractor shall prior to commencement of the Work,

 prepare and implement a Quality Control Plan (QC Plan)

 submit the completed QC Plan to the Ministry Representative

The QC plan may be operated wholly or in part by a qualified Subcontractor or an independent agency/organization. However, the plan’s administration and the quality of the Work remains the responsibility of the Contractor. The Contractor shall also ensure that all workers are familiar with the Quality Control Plans, its goals, and their role under it, as well as with the Contract specifications associated with the Work they are to undertake.

The quality control plan must at minimum contain the following information:

 Contractors basic approach to QC

 Contractors organizational structure

 Name, designation, and qualifications of the QC Manager that will sign project reports, assurance statement and as-built drawings.

 A listing of all other QC staff (including names, qualifications and relevant experience) and their assigned roles.

 A list of equipment to be used for the work.

 Quality control checklists for each element of work within the contract.

 Record keeping

 Non-conformance process to be used

 Methods that will employed to achieve objectives

 Layout standards / methods used

 Proposed location of aggregate sources along with applicable test results

 Aggregate production quality control methods

 Method for placement of materials

 Daily summary reports

 Photos of various QC items

 DAILY REPORTS MUST BE SUBBITTED ON A DAILY BASIS TO THE MINISTRY REPRESENTATIVE UNLESS OTHERWISED APPROVED BY THE MINISTRY.

2.13.4. Quality Control Final Report

The Contractor must submit a Final Quality Control Report that includes the following items:

 Mill Certificates

 Inspection Reports

 Completed checklists

 Any other relevant Quality Control items

The Contractor shall ensure that subcontractors and suppliers provide documented test results to confirm that specifications are met.

2.13.5. Payment Item 1.03

Section 2.13 will be paid for under Payment Item 1.03 on the Schedule A of the Major

2.14. Structure Certification

The Ministry will be responsible for the Bridge Certification (POR)

The Contractor MUST submit a detailed work schedule so that the Ministry’s POR can be on site for all the critical phases as indicated in the FS137. Any changes in the Work Schedule must be submitted and approved 72 hours before of any critical phase. Failure to provide adequate notice and obtain approval could result in all works being halted at the Contractor’s own expense until the POR can arrive at the site. If any of the critical work is conducted without the approval of the POR, then those works could be rejected at the discretion of the Ministry.

2.15. Final Payments

Final payments for the bridge are dependent on receiving:

 the Quality Management Package including:

› Quality Control Plan

› Quality Control Final Report

› Other Quality Control documents as per Section 2.13

FAILURE TO PROVIDE THESE ITEMS WILL RESULT IN WITH HOLDING PAYMENTS UNTIL ALL THE REQUIRED DOCUMENTS ARE SUBMITTED AND ACCEPTED BY THE MINISTRY.

Any incompletion, errors and omissions must be corrected by the contractor at no expense to the ministry prior to final payments on this project.

3. CONSTRUCTION SPECIFIC PROVISIONS FOR G6-070

3.1. Project Information

This Project involves the installation of a Ministry supplied double lane concrete slab bridge on spread footings. The Contractor is responsible for coordinating with the Ministry’s bridge fabricator for the delivery of the bridge and all its components. The cost of transporting of the new bridge to the site is at no cost to the Contractor but the Contractor is responsible for offloading the bridge.

The Contractor must also allow access through the site at all times. The Contractor will have to construct temporary access through the site to allow for public and industrial traffic. The Ministry has an 18m All-Steel Portable bridge that the Contractor can use but the Contractor will be responsible for the transporting of the temporary bridge to and from Forest District Yard in Mackenzie. The Ministry will also be responsible for the certification of any detour bridge that the Contractor installs.

3.2. Mobilization and Demobilizations

Mobilization and Demobilization shall be for all costs which are independent of the quantities of work done and which are not paid for under other items.

3.2.1. Payment with Item 2.01

Section 3.2 will be paid under Payment with Item 2.01 in Schedule A of the Major Works Contract.

 The first 50% of the Lump Sum price will be paid with the first progress payment once the equipment is on site.

 Payment of the remaining 50% of the Lump Sum price will be paid when all equipment has been removed from the site.

3.3. Temporary Access

3.3.1. Available Ministry All-Steel Portable Bridge

The ministry has an All-Steel 18m Portable Bridge available for use as the detour bridge.

The bridge is located in Mackenzie. The contractor may choose to use this portable, free of charge or the contractor may also choose to supply one of their own structures.

If the contractor decides to use the Ministry’s Portable, the contractor will be responsible for transporting the Portable to and from the yard.

3.3.2. Design and Construction of Temporary Access

Access through the site must be available at all times. The Temporary Access including;

Detour Crossing and Road must be adequately built to handle heavy Industrial and public traffic. The detour crossing and road must meet all requirements and design standards stipulated by:

 WorkSafe BC,

 MFLNRO,

 DFO,

 MOE and,

 any other applicable agencies.

The Detour Crossing and Road must be identified in the:

 Environment Management Plan

 Quality Control Plan

 Traffic Management Plan

 Safety Management Plan and,

 Work Schedule.

3.3.3. Deactivation of Temporary Access

The Temporary Access shall be deactivated and restored to original condition in accordance with the accepted Environmental Management Plan.

3.3.4. Payment with Item 2.02

Section 3.3 will be paid under Payment with Item 2.02 on the Schedule A of the Major Works Contract.

Any additional costs for maintenance, traffic control or other items to keep the Temporary Access adequate for the traffic will be considered incidental to the works and at no extra cost to the Ministry.

3.4. Excavation

Excavation includes all materials removed as necessary for the construction of foundations or other works.

 Foundation excavations shall not be larger than is reasonably necessary.

› Sub-excavation for the footing placement might be required. Sub-excavation of up to 1 meter in depth below design elevation will be considered incidental to the works. Any Sub-excavation beyond 1 meter of the design footing elevation will be considered a potential for extra works.

 Excavations shall be protected as necessary by barricades, shoring, dikes and/or cofferdams.

 Excavated material that the Ministry Representative deems unsuitable for backfill will be replaced with suitable material.

 For excavations in material other than rock, care shall be taken not to disturb the bottom of the excavation. If the bottom of the excavation is disturbed, the Contractor shall remove and dispose of all disturbed material and shall replace it with clean free draining pit run material placed in lifts not exceeding 200 mm compacted to the satisfaction of the Ministry Representative.

 Where concrete is to bear on rock, the rock surfaces shall be clean and free from any loose material.

 Where, in the opinion of the Ministry Representative, the bottom of an excavation is not competent, the Ministry Representative may direct the Contractor to excavate deeper.

The Ministry Representative may direct replacement of the incompetent material with clean free draining pit run material

 Excavation Safety will be consistent with WorksafeBC’s Regulations and The Company’s Safety Plan.

 Disposal of excess material to an approved location.

3.4.1. Payment with Item 2.03

Section 3.4 will be paid for under Payment with Item 2.03 in the Schedule A of the Major Works Contract.

3.5. Removal and Disposal of Existing Structure

The Contractor shall remove and haul the existing culvert(s) and/or bridge(s) off site to a recycling company or to an area near the site that is accepted by the Ministry. The site and method of disposal must comply with all Federal, Provincial, Municipal and Regional by-laws and regulations.

3.5.1. Payment with Item 2.03

Section 3.5 will be paid for under Payment with item 2.03 in the Schedule A of the Major Works Contract.

3.6. Stream Diversion

The Stream Diversion must meet all requirements stipulated by DFO, MFLNRO and MOE, and be identified as part of the Environmental Management Plan.

If a Stream Diversion is required for the works it will deemed incidental to the works and at no cost to the Ministry.

3.6.1. Payment with Item 2.03

Section 3.6 will be paid for under Payment Item 2.03 in the Schedule A of the Major Works Contract.

3.7. Stream Reconstruction

The Contractor shall re-construct and restore the stream bed in accordance with the Contract Drawings and the contractor’s Environmental Management Plan.

3.7.1. Payment with Item 2.03

Section 3.7 will be paid for under Payment Item 2.03 in the Schedule A of the Major Works Contract.

3.8. Rip Rap

The Contractor shall Supply, Haul and Place Rip Rap in accordance with the Standard Specification, Special Provisions and Contract Drawings.

Rip Rap is to be placed on a uniform even surface over a non-woven geotextile to dimensions as indicated on Contract Drawings.

 Place Rip Rap in a manner to secure and create a stable mass by:

 Use larger stones for lower courses, and as headers for subsequent courses,

 Stagger vertical joints and fill voids with rock spalls or cobbles, and

 Finish surface evenly, free of large openings, and neat in appearance

3.8.1. Payment with Item 2.04

Section 3.8 will be paid for under Item 2.04 in the Schedule A of the Major Works Contract.

3.9. Bridge Approach Granular Material

The Granular Material shall consist of sand/gravel (less than 75mm in size) and has to provide good drainage and smooth transition from the bridge approaches to the bridge structure. The Granular Material must meet the requirements in the Quality Control Plan and must be approved by the QC Manager and the Ministry. The Granular Material shall be placed in lifts not exceeding 300mm and the Contractor must have adequate equipment to ensure that the material is

compacted to 95% Standard Proctor Density or to the satisfaction of the Ministry Representative.

If required by the Ministry Representative the Contract must prove, at the Contractor’s expense, that the desired density of the material was achieved

3.9.1. Payment with Item 2.05

Section 3.9 will be paid for under Item 2.05 in the Schedule A of the Major Works Contract.

3.10. Bridge Construction

The Bridge Construction shall include Coordination with the Ministry’s Fabricator (Page 7), unloading and Installation of the Bridge, Signs, and No-post all in accordance with the Design Drawings. The cost of transporting the bridge from the fabricator to the site is NOT the responsibility of the contractor.

3.10.1. Welding

All welding shall conform to Section 4.13.2 of the Engineering Manual and Section 4.5 of the Forest Service Bridge Design and Construction Manual, both of which is in

accordance with CWB and CSA standards.

Welding shall be undertaken by a company approved by the CWB to the requirements of CSA W47.1, Division 3 or better. The Contractor shall produce evidence that all welding operators are currently qualified by CWB in the processes in which they are to be employed

3.10.2. Structural Field Grouting

Grouting shall be in compliance with Section 4.13.3 of the Engineering Manual. This section in the manual indicates criteria for Structural Field Grouting, Cold Weather and Hot Weather grouting procedures.

3.10.3. Special Considerations for Bridge

No special considerations.

3.10.4. Payment with Item 2.06

Section 3.10 will be paid for under Item 2.06 in the Schedule A of the Major Works Contract.

3.11. Revegetation

The Contractor shall supply and apply a certified grass seed mix and fertilizer. The grass seed mix must be addressed in the EMP and should conform to the following specifications or be approved by the Ministry Representative. Evidence of the certified seed mix and specifications shall be provided to the Ministry Representative upon request.

Grass seed shall be applied at a rate of 40 kg per hectare. A balanced fertilizer (18-18-18) shall be applied at a rate of 30 kg per hectare. Seeding and fertilizing should closely follow the final trimming and landscaping, weather permitting.

Disturbed areas within the Riparian Management areas shall be revegetated, by seeding with an approved grass/clover mix, mulching with hay/straw bales and the planting of willow shoots, as directed by the Ministry Representative or the EMP. Fertilizer must not be used within the Riparian Management Areas.

Seed and fertilizer application may be as a dry or wet application.

3.11.1. Payment with Item 2.07

Section 3.11 will be paid for under Item 2.07 in the Schedule A of the Major Works Contract.

3.12. Site/Pit Cleanup

At the conclusion of the works, the Contractor must remove the Contractor’s surplus products, tools, construction machinery and equipment not required for the performance of the remaining work.

The Contractor must also remove waste products and debris other than that caused by the Ministry, other Contractors or their employees, and leaves the work site clean and suitable for occupancy or use by the Ministry, unless otherwise specified.

The Bridge Sites, Pits and Waste Areas must be properly cleaned up to the satisfaction of the Ministry Representative.

Site/Pit Cleanup includes but is not limited to cleanup of slopes, waste piles, gravel pits, quarries, contractor camp ”if applicable” and the supply and installation of signs.

3.12.1. Payment with Item 2.07

Section 3.12 will be paid for under Item 2.07 in the Schedule A of the Major Works Contract.
