Cisplatin 유발 급성신부전에서 대식세포의 역할은 무엇인가?

배경 : 강력하고 효과적인 항암제인 cisplatin은 그 장점에도 불구하고 빈번한 신 독성으로 사용에 제한이 많아 그 병태생리의 규명 은 매우 중요하다. 최근 들어 cisplatin 유발 급성 신부전 발병기전에 염증 반응들이 관여한다는 연구 결과가 여럿 발표되어 본 연구 자들은 cisplatin 유발 급성 신부전에서 대식세포의 역할을 규명하고자 하였다. 방법 : cisplatin 유발 급성 신부전은 cisplatin 30mg/kg 을 마우스(C57BL/6) 복강 내로 주입하여 유도하였다. 신기능은 혈청 크레아티닌치와 BUN치로 대조군 (생리식염수), cisplatin 투여 1, 2, 3일째에 측정하였고 동시에 cisplatin 투여 후 대식세포 침윤 정도를 관찰하기 위해 신조직에서 CD11b 양성 세포 를 면역형광염색법으로 관찰하였다. 또한 ciaplain 유발 급성 신부전의 예방이 가능한 caspase-1 결핍 마우스를 이용하여 대식세포 침윤 정도를 대조군 및 wild type-cisplatin 유발 급성 신부전군과 비교하였다. 마지막으로 clodronate 투여 후 신기능 차이를 대조군 과 cisplatin 유발 급성 신부전군에서 비교하였다. 결과 : 1) 혈청 크레아티닌치와 BUN치는 cisplatin 투여 3일째에 정점을 보였다.

2) 대식세포 침윤은 cisplatin 주입 2일째부터 의미 있게 증가되어 급성 신부전 발생보다 선행됨을 관찰할 수 있었다. 3) Cisplatin 투여 후 대식세포 침윤은 wild-type보다 caspase-1 결핍 마우스에서 의미 있게 감소됨을 관찰할 수 있었다. 3) clodronate 치료는 cisplatin 유발 급성 신부전을 예방하지 못하였다. 결론 : Cisplatin 유발 급성 신부전에서 대식세포 침윤은 급성 신부전 발생보다 선행되었으나 clodronate투여는 cisplatin 유발 급성 신부전을 예방할 수 없었다.

A Case of Acute Arsine (AsH3) Intoxication

A 36-year-old male technician who handled gas in a semiconductor laboratory was admitted with severe abdominal pain and dark-red-colored urine. He was exposed to gas while changing a gas bomb. He mistook an arsine bomb for another bomb. The gas had a garlic odor. About 30 minutes after gas exposure, the patient suffered colicky abdominal pain, nausea, vomiting and dizziness. Arsine causes severe intravascular hemolysis with a rapid drop in hematocrit and hemoglobin levels. Rapid hemolysis may lead to oliguric renal failure and death. Other body organs can be involved as a result of the hypoxic effect of anemia and hemolysis, or as direct toxic effect of the arsine itself. On hospital in-patient day 2, the 24 h urine arsenic level was >1000 ug/day, and the serum arsenic level was 309.4 ug/L. Since the BUN/creatinine level had risen to 33/2.0 mg/dL at this stage, a hemoperfusion was performed. Hydration was then used to maintain urine output at 2 ml/kg/h. The patient condition improved, although he complained of a severe headache which is a neuropathic sequela of arsenic exposure. He was discharged with a normal blood arsine level. To our knowledge, this is the first report of arsine poisoning in Korea. Cases of arsine poisoning have been reported in other countries.
