크론병 환자에서 소장 점막하 해면성혈관종 출혈로 인한 소장절제 1예

서론: 상세 불명의 장출혈은 상부위장관 내시경과 대장내시경 검사를 시행해 원인을 찾지 못하는 경우로, 위장관 출혈로 내원한 환자 중 5%
가량이 원인을 찾지 못하고 캡슐 내시경, 혈관 조영술, 소장내시경 등을 시행하게 된다. 성인에서 상세 불명의 위장관 출혈의 흔한 원인으로 는 혈관확장증, 미란, 궤양등이 있으며 드물게 염증성 장질환, 장관의 신생물, 혈관종 등이 보고되고 있다. 본 사례에서는 크론병 환자에서 소장 점막하종양에 의한 위장관 출혈 의증으로 수술적 절제하여, 조직검사에서 해면성혈관종 확인되어 보고하는 바이다. 증례: 40세 남자가 혈변을 주소로 내원하였다. 환자는 7년 전 혈변으로 내원하여 검사하였으나 출혈소 찾지 못하고 증상 호전되어 퇴원했다가 10개월 후 다시 혈변으로 내원하여 대장내시경에서 출혈은 없으나 회맹판 궤양 발견하여 크론병 진단되었고, 캡슐내시경에서 말단 공장의 점막하종양 발견, 이로 인한 출혈 가능성 추정 하였으나 보존적 치료로 증상 호전되어 메살라진 유지중이었다. 금번 내원 당시 활력증후는 안정적이었으나, 급성 병색 보이며 결막은 다소 창백하였다. 복부 강직이나 압통, 반발압통 등의 이상소견은 없었으며 흉부의 이상소견은 없었다. 혈색소 내원 당시 10.6 mg/dL이었으나 출혈 지속되며 7.8 mg/dL까지 감소하여 계속적으로 수혈하였고, 상부위장관 내시경과 결장내시경 및 혈관조영술에 서 출혈소 발견하지 못하였다. 대장내시경 시행하여 회장까지 관찰하였으며 점막의 염증소견 없이 혈흔만 보이는 상태로 크론병에 의한 출 혈보다는 회장 상부의 출혈이 의심되었고 이전 발견했던 점막하종양의 출혈 의심 하 소장내시경 시행하였다. 상부 접근 소장내시경에서 병 변 없었고, 하부 접근 소장내시경에서 근위부 회장의 점막하종양 확인, 인디안 잉크로 표지하고 외과에서 소장 부분절제 했으며 조직검사로 해면성 혈관종이 확인되었다. 수술 후 경과 양호하며 퇴원했으며 경구 메살라진 복용하며 외래 추적 관찰 중이다.

Inhibitory effect of eupatilin on NSAID induced small intestinal mucosal injury in the rat

Objective: This study aimed to reveal the mucosal protective effect of the eupatilin isolated from Artemisia princeps on the rat small intestine.
Methods: Twenty-four pathogen free Sprague-Dawley rats were divided into four groups: one group without any control; another with indomethacin control; a third with eupatilin 30 mg/kg; the fourth with eupatilin 60 mg/kg. The rats given the intragastric route at the eupatilin for six days before indomethacin treatment. These were given indomethacin 200 mg/kg and sacrificed two days later. The effects of eupatilin were evaluated on macroscopic, histologic injury, inflammatory markers, plasma prostaglandin E2 (PGE2), plasma tumor necrosis factor alpha (TNFα), tissue myeloperoxidase (MPO) were measured by enzyme-linked immunosorbent assays. Results: The mean number of damage per rat was 5.5 for the indomethacin control group, 2.5 for the low dose eupatilin group, 1 for the high dose eupatilin group. The area of mucosal defect in a rat small intestine was 12.5±8.04 mm2 for indomethacin control group, 3.5±2.07 mm2 for low dose eupatilin group, 1.0±0.63 mm2 for high dose eupatilin group. Indomethacin decrease basal PGE2 (p<0.002), but PGE2 were not modified by eupatilin (p=0.394, p=0.132). TNFα of indomethacin control group were 29.47±5.37 pg/ml. TNFα level induced by indomethacin was significantly reduced by low dose eupatilin (p<0.001) or high dose eupatilin (p<0.001). MPO of indomethacin control group were 8.3±4.43 U/L. MPO induced by indomethacin was significantly reduced by low dose eupatilin (p<0.013). Conclusion: The study could conclude that eupatilin in rat small intestinal of models significantly decreased the macroscopic damage, plasma TNFα and tissue MPO, whereas eupatilin slightly affected the infiltration of tissue neutrophil and plasma PGE2. These suggests that eupatilin can be used as a mucosal protective agent in the small intestinal injury by the indomethacin.
