미만성 B형 대세포 림프종에 의한 공장 천공 1예

소장은 종양의 발생이 적어 소화기계 악성종양 중 소장의 악성종양이 차지하는 비율은 2%정도이고, 특히 소장의 악성 림프종은 위장관 악 성종양의 1% 미만을 차지할 정도로 드문 질환이다. 또한 초기 증상이 복통, 체중감소 등으로 비특이적이므로 질환이 진행하여 장폐쇄, 장출 혈, 장천공 등의 합병증을 유발한 이후에 진단되는 경우가 보고 되고 있다. 79세 남자가 하복부 동통으로 내원하여, 보존적 치료 도중 자발적 으로 장 천공이 발생하였다. 개복 수술 후, Treitz 인대로부터 20cm 하방에 공장 천공을 동반한 9.5x6cm 크기의 종괴가 있어, 소장의 segmental resection을 시행하였다. 종괴는 미만성 B세포 악성 림프종으로 진단되었다. 환자는 수술 후 CHOP 항암 화학 요법을 6차에 걸쳐 시행하였고 현재 완전 관해 상태로 경과 추적 중이다. 저자들은 공장 천공으로 응급 개복 수술 후 진단된 미만성 B형 대세포 림프종 1예를 경험하였기에 보고하는 바이다.

A case of primary gastric marginal zone B-cell lymphoma presenting as multiple lymphomatoid polyposis

Marginal zone B-cell lymphoma of the gastrointestinal tract most commonly affects the stomach. Endoscopic findings of primary gastrointestinal lymphoma are much various and heterogeneous. Marginal zone B-cell lymphoma is most commonly located in the antrum, 41%

but may be multifocal in 33% of cases. Lesion may appear as ulcers in 47% of cases, erosions in 23%, or as erythema in 30%. Gastrointestinal lymphomas presenting multiple polyposis on endoscopy are rare. There is no case of marginal zone B-cell lymphoma manifesting as multiple polyposis in stomach, although a few cases have been reported in colon. We present a rare case of 77-year-old female patient diagnosed as primary gastric marginal zone B-cell lymphoma presenting as multiple lymphomatoid polyposis. Several small lymph node infiltrations were noted along the lesser curvature of stomach. For absence of Helicobacter pylori and disseminated pattern of disease, we decided to treat by combination chemotherapy. Symptom of patient, polyposis lesions on endoscopy and lymph node infiltration on abdomen CT disappeared after treatment of six months.
