<How to Write Application Form>
1. The registration number in the form should be filled by the registration manager after signing in the registration book
2. Section ①: Mark ✔ on the box □ of the level of test you are applying for
*TOPIK I: Beginners, TOPIK II: Intermediate/Advanced (Mark only one) 3. Section ②: Registration number will be given by the registration staff according to its regulation.
4. Section ③: Write a testing area of country you are appying for
5. Your photo must be a size of 3 X 4cm, taken within 3 months; attach one on the application form and the other on the registration slip.
※ Photos must NOT be: Black and white, unclear, with any cap (graduation cap), with sunglasses, earphones, etc., covering face with fringe, facing camera in up, down, left or right angles or someone else’s photo
6. Section ④: Write your surname and given name written in Korean phonetically.
7. Section ⑤: Write your surname and given name written both in capital letters as it is written in official identification documents. For section ⑥, mark ✔ on the box of your gender.
8. Section ⑦: Write your nationality in Korean or English. For overseas Korean, please mark ✔ on the applied ( ).
9. Section ⑧: Mark ✔ on the box of your occupation.
10. Section ⑨: Write the date of your birth in (yyyy/mm/dd) form, and write your age in ( ).
11. Section ⑩: Write your address and contact details.
12. Section ⑪: Mark ✔ on the box of your channel for knowing TOPIK 13. Section ⑫: Mark ✔ on the box of your purpose of taking TOPIK
<How to Fill out Test Registration Slip>
1. Write Section ⑬ to ⑳ same as the application form.
2. You(Test Taker) must bring the registration slip to the Test day
