Abstract — Cardiac myocytes are subjected to fluid shear stress during each contraction and relaxation. Under pathological conditions, such as valve disease, heart failure or hypertension, shear stress in cardiac chamber increases due to high blood volume and pressure. The shear stress induces proarrhythmic longitudinal global Ca2+ waves in atrial myocytes. In the present study, we further explored underlying cellular mechanism for the shear stress-induced longitudinal global Ca2+

wave in isolated rat atrial myocytes. A shear stress of ~16 dyn/cm2 was applied onto entire single myocyte using pres- surized fluid puffing. Confocal Ca2+ imaging was performed to measure local and global Ca2+ signals. Shear stress elicited longitudinally propagating global Ca2+ wave (~80µm/s). The occurrence of shear stress-induced atrial Ca2+ wave was elim- inated by the inhibition of ryanodine receptors (RyRs) or inositol 1,4,5-trisphosphate receptors (IP3Rs). In addition, pre- treatment of phospholipase C (PLC) inhibitor U73122, but not its inactive analogue U73343, abolished the generation of lon- gitudinal Ca2+ wave under shear stress. Our data suggest that shear-induced longitudinal Ca2+ wave may be induced by Ca2+-induced Ca2+ release through the RyRs which is triggered by PLC-IP3R signaling in atrial myocytes.

심장 주기 동안 발생하는 심장 내의 기계적 환경 변화는 심장 의 흥분성과 수축력을 변화시킨다.1-3)밸브질환, 고혈압, 심부전 과 같은 병변 상태에서 심방의 압력과 부피의 증가는 심방 흥분 성을 변화키는 중요한 요인으로 여겨져 왔다.1-3) 이러한 병변 상 태에서 신장(stretch) 및 심방 부피 증가가 관찰되고, 신장에 의 해 활성화되는 이온채널(stretch-activated ion channel, SAC)이 이러한 심방 기능변화를 매개할 것으로 제안된 바 있다.4-8)심장 이 매번 수축할 때, 또 혈류동역학적 장애가 발생할 때에는 혈류 에 대해 근세포 층들 간의 움직임이 발생되고, 이로 인해 심근세

포는 전단 자극에 노출된다.9,10)특히 승모판 부전 환자에서는 혈 액의 역류 분사가 심방 세동을 일으킨다고 보고되었다.2)그러나 아직까지 전단 자극에 대한 심방의 반응은 잘 밝혀지지 않았다.

심장근세포에 전단 자극 약 16 dyn/cm2를 가하면 세포 종방향 (longitudinal: 길이 방향)으로 전파되는 Ca2+웨이브가 발생된다.11) 심근세포의 Ca2+웨이브는 세포막에 존재하는 Ca2+의존성 이 온 수송체 단백질의 기능을 변화시켜 세포 막전위에 영향을 미 칠 수 있다. 최근 전단 자극이 심근세포의 L-type Ca2+채널을 억제시키고, 전압의존성 K+채널의 세포막 이동을 촉진한다고 보고된 바 있다.12,13)흥미롭게도 이러한 전단 자극 반응은 세포 내 Ca2+증가를 억제하였을 때 사라진다. 그러나 아직까지 전단 자극에 의한 Ca2+증가의 원인 기전은 모호하다. 본 연구에서는 이 전단 자극에 의해 유도되는 Ca2+웨이브가 어떠한 기전으로 발생되는지 알아내고자 하였으며, 이를 위해 이차원 공촛점 Ca2+

영상화 기술 및 단일세포 전단 자극 처리 기술을 접목하였다.

[Ca2+]-Tyrode 용액에 collagenase A(1.4 mg/ml, Roche)와 protease(0.14 mg/ml, Type XIV, Sigma)를 포함한 용액을 12분간 관류시키고, 마지막으로 0.2 mM[Ca2+]을 포함한 Tyrode 용액을 5분간 관류시켜 사용된 효소를 제거하였다. 심장을 랑겐도르프 장 치에서 분리한 후 심방을 취하여, 기계적 방법으로 세포들이 떨 어져 나오게 하였다. 분리한 세포는 실험에 사용하기 전까지 실 온에서 0.2 mM[Ca2+]을 포함한 Tyrode 용액에 보관하였다.

전단 자극의 처리

세포에 계속 2 mM CaCl2를 함유한 Tyrode 용액을 흘려주었 다(pH 7.4). 40 cm 높이의 용액 저장고와 연결된 미세관 끝(내 경 250 μm)을 세포로부터 ≈150 μm 거리에 두고, 이 미세관을 통해 압력을 갖는 유체를 단일 세포에 적용시켰다.13) 미세관 끝 은 챔버 바닥에 닿아 한 쪽을 향해 45o각도로 기울여 놓았다.

자동 제어가 가능한 솔레노이드 밸브를 용액 저장고와 미세관을 연결하는 튜브 가운데에 설치하였다. 원통형 튜브안의 유체가 갖 는 전단력(dyn/cm2)은 다음 식을 통해 계산되었다.14)

여기서 μ는 유체의 점도(물은 1.002 · 10−2dyn · s/cm2), Q는 유 속(cm3/s), r은 미세관의 내경(cm)을 각각 나타낸다. 이 마이크 로-젯 장치는 용액 저장고의 높이가 40 cm일 때 ~16 dyn/cm2 (0.16 N/m2와 같음)의 전단력을 만들어 낸다. 미세관의 위치를 조 절할 때는 미세조절장치(micromanipulator; Prior England 48260)를 사용하였다. 실험 전 현미경과 영상 모니터를 통해 전 단 자극 시 세포가 움직이지 않음을 확인하였다. 실험은 실온에 서 진행하였다(22~25oC).

이차원 공초점 Ca2+영상화 및 영상 분석

영상화 실험에는 60배 oil-immersion 대물렌즈(Plan Apo, 개 구수 1.4, Nikon, 일본)가 장착된 도립현미경(Ti, Nikon)이 연결 된 레이저 스캔 공초점 현미경(A1, Nikon)을 사용했다.15) Ca2+

통계 처리

실험 데이터의 수치들은 평균±표준오차(standard error of mean, SEM)로 나타내었다. 통계학적인 비교는 “paired Student’s t test”로 이루어졌다. P 값이 0.05 미만일 때 통계학적으로 유의 한 차이가 있는 것으로 간주 하였다.

실험결과 및 고찰

전단 자극에 의해 유도되는 종방향의 심방세포 Ca2+ 웨이브와 이에 대한 tetracaine의 효과

흰 쥐 심방근세포에서 전단 자극은 종방향의 Ca2+웨이브를 유발한다고 알려져 있다.11) 흰 쥐 심방근세포에서 전단자극(~16 dyn/cm2)은 이전 보고와 일관된 길이 방향 Ca2+웨이브를 유발 시켰다(Fig. 1A, 2A; ‘Con, shear’). 이 Ca2+웨이브의 평균 Ca2+

이동 속도는 79.7±7.69 μm(39 세포)이었다. 8초의 전단 자극 처 리 시간 동안, 평균 1.23±0.26번의 Ca2+웨이브가 관찰되었다 (39 세포). 전단 자극 처리 시점부터 웨이브의 발생 시점까지는 0.2~3초 정도의 지연기(delay time)가 있었으며, 이 값은 세포마 다 다소 차이가 있었다. 어떤 세포에서는 두 개 이상의 국부 위 치에서 웨이브가 교대로 시작되어 전파되는 모습을 관찰할 수 있 었다(Fig. 2A, ‘Con, shear’). 종방향 웨이브가 전파된 영역에서 국부 Ca2+농도(ΔF/F0)의 평균 증가량은 4.9±0.57이었다(39 세 포). 전단 자극 시 심방 세포에서 발생되는 길이 방향의 Ca2+ 이브는 매우 느린 속도의 세포 Ca2+농도 증가와 비교적 지속적 인 Ca2+증가를 동반하였다(Fig. 1B, black trace). 실제 심방 근 세포에서는 활동 전위 시 횡방향으로 전파되는 빠른 속도의 Ca2+

웨이브(~200 μm/s)가 생리학적으로 발생된다.16)이 웨이브와 비 교했을 때, 전단 자극에 의해 유발되는 느린 웨이브는 시공간적 성격이 다르고, 심방 흥분-수축 연결(excitation-contraction coupling) 과정을 변형시킬 가능성이 크다.

심방 근세포에 주로 존재하는 Ca2+유리 채널은 ryanodine

receptor(RyR)이며, 이 단백질들은 세포 전체에 걸쳐 규칙적으로 분포하고 있다.17,18)전단 자극 시 발생되는 Ca2+이온의 전파가 이 단백질들의 개폐에 의한 것인지 고찰하고자, 이 단백질에 대

한 선택적 억제제인 tetracaine19)을 사용하였다. 한 세포에 비슷 한 세기의 전단 자극을 3~4분 간격으로 다시 적용 시, 비슷한 양상의 Ca2+웨이브가 발생되었으므로 같은 세포에서 Ca2+

부 근소포체(sarcoplasmic reticulum; SR)의 막에 IP3R2가 RyR 과 공존한다고 알려져 있다.17,18)따라서 IP3R를 통한 Ca2+유리 가 Ca2+-induced Ca2+ release(CICR)에 의해 이웃한 RyR의 활 성 증가를 야기할 수 있다.18)전단 자극 시 Ca2+웨이브 발생이 IP3R을 통한 Ca2+유리에 의하는지 알아보기 위해, 이 단백질에 대한 억제제 2-aminoethoxydiphenyl borate(2-APB)를 사용하였 다. 이 약물은 심방 근세포 IP3R를 2~5 μM 농도에서 선택적으 로 억제한다고 알려져 왔다.18)전단 자극 하에 길이 방향 Ca2+

웨이브가 기록된 세포에서, 2 μM 2-APB를 8~10분간 처리하고, 다시 전단 자극의 효과를 고찰하였다. 이 약물을 처리한 후에는 전단 자극 시 Ca2+웨이브가 발생되지 않았다(Fig. 2). 전단 자 극에 의해 유도된 Ca2+웨이브는 2-APB의 제거 후에 다시 회복

침해 준다.

전단 자극 시 발생되는 심방 Ca2+웨이브에 대한 phospholi- pase C(PLC)의 역할

위 실험 결과로부터 얻은 가설을 더 검증하기 위해, IP3를 생 산하는 PLC가 전단 자극에 의한 Ca2+웨이브 발생에 기여하는 지 알아보았다. PLC 억제제 U73122를 전처리(5 μM, 5분) 한 후 전단 자극을 가하였을 때, 길이 방향으로 전파되는 Ca2+웨이브 의 생성은 억제되었다(Fig. 3). 이와 달리 U73122 구조와 유사하 나 PLC를 억제하지 않는 약물 U73343(5 μM, 5분)을 처리하였 을 때엔, 전단 자극에 의해 여전히 길이 방향의 웨이브가 관찰되 었다(Fig. 4). 이러한 결과들은 전단 자극 시 Ca2+웨이브의 발

생이 PLC의 활성화에 의함을 나타낸다.

결 론

심방 근세포에서 전단 자극에 의해 유도되는 종방향 Ca2+ 이브는 오래전 보고된 바 있으나, 아직까지 그 발생기전에 대한 이해는 부족하였다. 본 연구 결과는 전단 자극이 심방세포에서 PLC-IP3R 신호전달 경로의 활성화를 일으키고, 이로 인해 RyR 을 통한 Ca2+유리가 발생하여 종방향의 Ca2+웨이브가 발생됨 을 보여준다.

전단 자극 시 증가되는 Ca2+양은 세포막에 존재하는 Na+- Ca2+교환체를 활성화 시켜 막전압의 탈분극을 야기할 수 있으 며, 이는 부정맥 발생의 주요 원인이 된다. 최근 본 연구그룹에 서 전단자극 시 Ca2+의존적으로 transient receptor potential melastatin 4(TRPM4) 채널이 활성화됨을 관찰하였고, 이 또한 막 전압의 탈분극 및 활동전위의 변형을 초래할 수 있어, 부정맥 발 생 및 흥분-수축 연결 과정의 현저한 변화를 초래할 것으로 예상 된다. 전단 자극 시 이러한 Ca2+웨이브를 매개하는 PLC의 상위 (upstream) 신호전달 과정은 앞으로 연구될 필요가 있을 것이다.
