Chronic kidney disease (CKD) is a worldwide problem. The overall prevalence of CKD in Korea is estimated to be 13%, and CKD is a major risk factor for the cardiovascular events and all-cause mortality. The socioeconomic burden of CKD is alarming, where total costs associated with CKD are estimated over KRW 5,000 billion in 2011. However, public awareness regarding the significance of CKD is insufficient. Korea Centers for Disease Control and Prevention (KCDC) is making great effort to prevent CKD and to educate primary physician, making the clinical guideline and a large-scale CKD cohort.

The KNOW-CKD (KoreaN Cohort Study for Outcome in Patients With Chronic Kidney Disease), funded by KCDC, was established in 2011 to investigate the natural course of CKD and to evaluate the etiologic factors for renal progression with related complication.

It is a patient-based cohort study. Total 3,850 subjects from 17 centers have been enrolled and biobank for 24,290 samples have been constructed for five years. The subjects were classified into subgroups based on the specific causes of chronic kidney disease – i.e., glomerulonephritis, diabetic nephropathy, hypertensive nephropathy, polycystic kidney disease. Pediatric CKD and CKD with functioning kidney allograft were also included in the CKD cohort. Subjects were followed up every year and laboratory data and medical history were collected.

Clinical practice guideline about CKD for primary physician was developed by KCDC in 2012. This guideline includes the definition, classification and management for CKD. The section is divided into diabetic nephropathy, hypertension, glomerulonephritis, polycystic kidney disease and pediatric kidney disease. Pathophysiology, diagnosis and management for each subgroup and when to refer to nephrologist are discussed in detail.

In this lecture, we will briefly show the progress of the KNOW-CKD and look into the clinical guideline.
