서론

패혈증은 감염증에 대한 숙주의 전신 염증반응에서 기인

하는 생리적, 병리적, 생화학적 이상과 관련된 증후군을 지

칭한다[1]. 패혈증은 높은 중증도 및 사망률의 주요 원인으

로 전 세계적으로 높은 병원비와 입원일수 증가에 영향을

미친다[2-4]. 1991년 미국호흡기학회(American College of

Chest Physician)와 중환자의학회(Society of Critical Care

Medicine)의 합의 회의를 통한 패혈증의 표준화된 정의와

번들 치료를 도입한 이후 20여 년이 지난 현재 패혈증으로

인한 사망률은 20%로 여전히 높다[4-6]. 2016년 미국중환

자학회와 유럽중환자학회(European Society of Intensive

Care Medicine)는 패혈증과 패혈성 쇼크에 대한 3차 국제

적 표준 정의 및 국제 가이드라인을 개정하였으나[7,8], 이

후 패혈증의 정의와 패혈증 환자의 치료에 대해서는 현재까

지도 많은 논란이 있다[9-11]. 본 논문은 패혈증 치료에 있

어 적절한 항생제 치료법과 항생제 스튜어드십 적용에 대해

서 논의하고자 한다.

The management of sepsis and septic shock remains challenging. The aim is to apply the optimal antimicrobial therapy and antimicrobial stewardship to patients in state of sepsis or septic shock. To reduce the mortality of sepsis and septic shock, it is critical to promptly administer the appropriate antibiotics with an accurate diagnosis. De-escalation is needed 48 to 72 hours after the first administration of antibiotics depending on the findings of causative pathogens. In the case of antibiotic resistance, the importance of an antibiotic stewardship program is increasingly being emphasized. Antimicrobial stewardship implies coordinated interventions designed to improve the appropriate use of antibiotics by promoting the selection of an optimal drug regimen such as dosing, duration of therapy, and route of administration. An antibiotic stewardship program may also be applied to patients of both sepsis and septic shock. Efforts such as the selection of appropriate empirical antibiotics, de-escalation, and determination of whether to stop antibiotics with procalcitonin may improve the clinical prognosis of patients with sepsis as well as the successful implementation of an antibiotic stewardship program.

패혈증의 초기 항생제 치료

패혈증의 초기 항생제 치료는 적절한 항생제의 선택, 항

생제 투여 시간, 단일 또는 병합 요법, 항생제 약동학적 특

성 등 여러가지 측면을 고려해야 한다. 패혈증 치료에 있어

사망률을 유의하게 감소시키는 전략 중 하나는 적절한 항생

제 투약이다[12,13]. 패혈증에서 초기 경험적 항생제가 적절

하지 않을 경우 생존율이 5배까지 감소할 수 있다[14]. 패혈

증 환자에서 적절한 항생제를 선택하는 것은 복잡해서 정형

화된 알고리즘 등의 형태로 제시하기는 어렵다. 적절한 항생

제 선택을 위해서는 감염병소와 추정 원인 균, 지역의 균주

내성현황, 숙주의 면역 상태와 기저질환, 숙주의 다제내성균

보균 가능성 등의 기준을 종합하여 결정하는 것이 필요하다

[15]. 지역의 균주 내성현황은 시대에 따라 변할 수 있기 때

문에 정기적인 추적관찰이 필요하다.

패혈증에서 항생제를 시작하는 시점은 중요하다. 가이드

라인에서는 패혈증 및 패혈성 쇼크를 인지하는 시점에 가능

한 빨리 정주 항생제를 1시간 이내에 투약할 것을 권고하

고 있다[8]. 실제로 패혈증 환자에서 초기 항생제 투약시간

이 매시간 늦어질 때마다 사망률이 유의하게 증가하고 입원

일수 증가 및 급성 신손상, 급성 폐손상의 위험이 높아진다

[16-19]. 그러나 패혈증에서 반드시 1시간 이내 투약이 이

루어져야 하는가에 대해서는 논란이 있다. 1시간 이내 항생

제 투약을 이루기 위하여 우리는 중요한 다른 목표 달성이

어려울 수 있다. 정확한 진단 없이, 배양검사를 시행하지 않

고, 원인균에 대한 고민이 없는 빠른 투약에만 초점이 맞춰

진 광범위 항생제 투약은 때로는 환자에게 적절하지 않은

항생제 투약이 이루어짐으로 오히려 나쁜 예후를 초래하면

서 항생제 내성을 증가시킬 수 있다. 또는 초기 수액공급과

승압제의 신속한 투약을 지연시킴으로 장기부전을 악화시

킬 수도 있다. 일부 연구자들은 패혈증의 항생제 치료에 있

어 1시간 이내의 항생제 투약시간을 준수하는 것보다는 패

혈증 번들을 준수하고 항생제 스튜어드십을 수행하는 것이

더 중요한 문제라고 주장한다[11,20-22].

항생제 병합요법의 경우 가이드라인에서는 패혈성 쇼크

환자에서 초기 경험적 항생제 치료로 서로 다른 계열의 항

생제 병합요법을 고려할 수 있지만 지속치료를 위한 병합요

법은 권고하지 않는다. 균혈증, 패혈성 쇼크, 호중구감소증

이 동반된 환자에서도 지속치료에 있어서 항생제 병합요법

은 예후 개선에 영향을 미치지 않는다[23-25]. 따라서 패혈

성 쇼크 환자에서 병합요법을 시작했다면 임상적 호전을 보

이는 수 일 이내 병합요법을 중단할 것을 권고한다. 단, 확

정적 치료로 병합요법이 필요한 경우(예: 다제내성균주 치

료, 항독소 치료)는 병합요법을 지속할 수 있으며 감염내과

의사의 자문을 받는 것이 도움이 될 수 있다.

패혈증의 항생제 요법에 있어

단계적 축소치료 전략

항생제 요법에서 단계적 축소치료는 초기 배양검사 및 항

생제 감수성 결과와 경험적 항생제의 임상반응을 바탕으로

한 좁은 범위의 항균제 선정과 감염증이 없음을 확인 후 항

균제 치료를 중단하는 것을 의미한다[26]. 단계적 축소치료

는 경험적 항균제 치료에서 사용되었던 광범위 항생제에 비

하여 다음과 같은 효과를 기대해볼 수 있다. 첫째, 항생제 내

성균 감소 전략에 도움이 될 수 있다. 경험적 항생제 치료목

적으로 사용되는 광범위 항균제의 장기간 사용은 내성균 증

가에 기여를 한다[27,28]. 내성균 감염의 증가는 감염증 치

료 자체를 어렵게 하고 병원비 및 병원 입원일수 증가에 영

향을 미치며 또다른 다제내성균의 발생을 유도한다[29,30].

둘째, 광범위 항생제 사용에 의한 항생제 관련 부작용의 감

소를 유도할 수 있다. 흔한 부작용의 예로는 Clostridioides

difficile 감염, 내성균 또는 진균에 의한 중복감염을 들 수 있

다. 마지막으로 단계적 축소 전략은 전체 항생제 사용과 비

용의 감소를 유도함으로써 전체 병원비와 입원일수 감소에

기여한다[28,31].

많은 관찰연구 및 무작위연구에서 단계적 축소치료 전략

은 안전한 것이 입증되었다[32-39]. 패혈증 환자를 대상으

로 한 연구에서 약 40%의 환자가 단계적 축소치료를 받았

다[36,40]. 모든 연구에서 단계적 축소치료는 사망률 증가

에 영향을 미치지 않았으나 한 연구에서 단계적 축소치료

는 중증 패혈증 환자에서 중복감염빈도 증가 및 입원일수

증가와의 관련성을 보였다[41].

항생제 단계적 축소치료를 위해서 반드시 선행되어야 하

는 것은 원인 균주를 찾기 위한 노력이다. 일단 항생제가

투여되기 시작하면 첫 번째 투약 이후 수분에서 수시간 내

에 배양검사에서 균이 분리되지 않을 수 있기 때문에 패혈

증 환자에서 배양검사는 반드시 첫 번째 항생제 투약 전

에 완료하는 것이 필요하다[42,43]. 배양검사를 위한 검체

는 무균법을 사용하여 감염이 있는 그 부위에서 충분한 양

을 획득하여 지정된 멸균 용기에 담는 것이 필요하다. 배

양검사는 특이도가 높고 항생제 감수성 결과를 알 수 있다

는 장점이 있지만 양성률이 낮고 결과 도출 시간이 비교적

느리다는 단점이 있다. 이러한 단점을 보완하기 위한 분자

진단 기법이 도입되고 있다. 대표적인 것이 MALDI-TOF

(matrix-assisted laser desorption/ionization

time-of-flight) 질량분석기로 기존의 배양검사와 비교하여 균주 동

정을 2시간 앞당겼고 현재 현장에서 상용화 되고 있다[44].

그 외에도 microarray, multiplex 중합효소연쇄반응 기법

을 이용한 균주 동정 및 내성유전자 확인법이 연구 및 적

용 중에 있다[45,46]. 이처럼 패혈증의 원인 균주를 신속하

게 확인하기 위한 노력은 항생제 단계적 축소 전략에 도움

이 될 수 있다.

패혈증 환자에서의 항생제 스튜어드십

항생제 스튜어드십은 항균제의 선택, 투약 용량, 투약 기

간과 경로를 포함한 적절한 항생제 사용을 촉진하는 조직

화된 행동양식으로 정의될 수 있다[47,48]. 항생제 스튜어

드십은 1996년 McGowan과 Gerding [49]에 의해서 처음

개념이 제시된 이후 1997년 미국의료역학회(Society for

Healthcare Epidemiology of America) 및 미국감염학회

(Infectious Diseases Society of America) 가이드라인에 항

생제 내성 예방에 포함되었다 이후 1998년 유럽임상미생

물감염학회에 연구그룹(the European Society of Clinical

Microbiology and Infectious Diseases Study Group for

Antimicrobial Stewardship)이 설립되면서 그 개념이 전세

계로 확대되었다. 2011년까지 항생제 스튜어드십은 크게 주

목받지 못하고 단일 기관이나 집단에서 이루어졌으나 무분

별한 항생제 사용 증가에 따른 다제내성균주가 문제가 되면

서 전세계적인 관심이 증가하였다. 항생제 스튜어드십은 크

게 두 가지의 목적을 가진다. 하나는 독성, 내성 획득 등의

항생제에 의한 의도치 않은 결과를 최소화하면서 환자의 임

상 호전을 유도하고, 두 번째는 의료의 질에 영향을 미치지

않는 범위 내에서 병원비의 감소를 목표로 한다[28].

항생제 스튜어드십 프로그램은 다음의 일곱 가지의 핵심

요소를 포함한다(Table 1) [50]. 항생제 스튜어드십이 병원

에서 적용되기 위해서는 감염전문의사, 임상약사, 임상미생

물 전문가, 감염관리 전문가, 역학전문가, 행정직을 포함한

구성원의 상호 노력이 필요하다. 항생제 스튜어드십의 성공

적인 수행을 위해서는 특히 임상의사가 단계적 축소를 실천

하는 것이 도움이 된다[26].

항생제 스튜어드십 수행은 항생제를 시작하는 순간부터

종료시점까지 고려해야 한다. 초기 경험적 항생제 투약 시

적절한 범위의 항생제를 선택하는 것, 초기 항생제 투약 후

48-72시간 뒤 원인균을 찾기 위한 검사결과에 따라 확정적

항생제 변경을 고려하는 것, 감염증에 따라 적절한 투약기간

후 중단을 고려하는 것 모두 항생제 스튜어드십 수행의 일환

이라 볼 수 있겠다.

항생제 스튜어드십은 패혈증 환자에서도 동일하게 적용될

수 있다. 패혈증 환자 치료에 있어 초기 항생제 선택에 있어

감염내과 의사의 자문을 받을 경우 초기 항생제 선택의 적절

성과 이후 단계적 축소 수행의 비율이 높아진다[51]. 아마도

감염내과 자문을 받게 되면 감염병소와 원인균을 찾는 노력

을 더 기울이게 되기 때문인데 이처럼 정확한 진단과 원인

균주를 찾는 노력은 환자의 임상예후 향상과 더불어 항생제

스튜어드십 수행에도 도움이 된다.

패혈증 환자의 조기진단과 항생제 스튜어드십 수행을 위

하여 분자 진단기법이 시도되고 있다. 현재 연구 중인 생체

지표에는 급성 반응 물질(C-reactive protein [CRP],

pro-calcitonin [PCT], lipopolysaccharide-binding protein,

pentraxin, amyloid A), 사이토카인(interleukin [IL]-6,

IL-8, IL-10), 응고지표(protein C, biphasic waveform)

등이 있고 대표적인 두 가지가 CRP와 PCT이다[52]. CRP는

세균감염 및 패혈증 환자에서 높은 민감도를 보이나 특이도

가 낮다는 단점이 있다[53]. CRP는 단독 지표로서 패혈증이

의심되는 환자에서 세균감염 동반 여부를 배제하고 예후를

예측하는 데에는 한계가 있다[54,55]. PCT는 단독 지표로서

CRP보다 패혈증 환자의 진단에 있어 특이도가 높고 패혈증

환자의 예후를 예측한다[56]. PCT는 패혈증 환자에서 항생

제 스튜어드십 수행에 유용한 지표로 자리잡았다[57-59].

개정된 패혈증 가이드라인에서는 PCT를 패혈증 환자에게서

항생제 유지기간을 결정하고 감염증이 동반되지 않은 환자

에게서 항생제 중단의 지표로 사용해볼 수 있다고 권고하였

다[8]. 항생제 중단을 고려할 수 있는 PCT의 기준은 전 세계

적으로 표준화되지 않은 상태이나 통상적으로 0.5 ug/L 미

만일 경우 항생제 중단을 고려해볼 수 있다[60]. PCT를 이

용한 항생제 스튜어드십은 패혈증 환자에서 사망률에 영향

을 미치지 않으면서 항생제 치료의 총 지속기간을 4일 감소

시키는 결과를 확인하였다[61,62]. 그러나 PCT를 이용한 항

생제 스튜어드십은 중증 패혈증 환자에게는 신중히 적용해

야 한다.

결론

패혈증 환자의 사망률 개선을 위해서는 신속한 진단과 치

료가 필요하고, 그 중 적절한 항생제 투약을 신속하게 투약하

는 것이 도움이 된다. 무분별한 광범위 항생제 사용은 전 세

계적으로 항생제 내성 증가를 초래하였고 이로 인하여 항생

제 스튜어드십 프로그램을 도입하게 되었고 개인, 기관, 범

세계적 측면에서 시행되고 있으며 패혈증 환자에게도 적용될

수 있다.

패혈증에서 항생제 사용은 임상적 측면과 항생제 스튜어

드십 측면을 모두 반영할 때, 패혈증 초기 단계에서 적절한

항생제의 신속한 사용, 신속한 미생물학적 진단에 따른 항생

제의 단계적 축소, PCT 등을 이용한 항생제 중단 시점의 결

정의 세 가지 전략이 도움이 될 수 있다.

이 논문은 패혈증 치료에 있어서 초기의 적절한 항생제 치료 원 칙에 대해 최근 지식을 정리하여 설명하고 있다. 대부분 질병은 진단 후 치료를 시작하나, 패혈증과 패혈성 쇼크는 감염장소와 원인 미생물을 확인하지 못한 상태에서 경험적으로 항생제를 선 택하게 되는 경우가 많다. 따라서 초기의 신속한 진단과 적절한 치료를 시행하는 것이 환자의 예후를 결정하게 된다. 패혈증과 그 원인을 신속하고 정확하게 진단하기 위한 최근의 연구자료, 초기에 적절한 항생제를 선택하는 전략, 진단 후 적절하게 항생 제를 유지/축소/중지하는 항생제 스튜어드십 전략을 정리하여 설명해 주고 있다. 이 논문은 패혈증 환자의 생존율을 높이는 것 뿐 아니라, 다제내성 세균의 출현을 막고, 항생제에 대한 이상 반 응을 줄이는 데에도 많은 도움을 줄 것으로 판단된다.
